ORDINARY PASSPORT
The passports are printed at the Ministry of Internal Affairs in Baku. Obtaining passports through the Consular Section of the Embassy usually takes 2-3 months.
Passport expired or pages are full: Required documents for issuance of a new passport
- Form (Download)
- Original passport (with full pages, expired)
- Original valid identification card (şəxsiyyət vəsiqəsi) or high resolution colored copy
- Original Military ID (hərbi bilet) or high resolution colored copy, for male applicants aged between 18-35. Or original document proving the exemption from military service or right to deferment;
- Crime report from the police if the passport has been stolen or lost
- 2 passport photos (3x4 or 3.5x4.5)
- Consular fee: 25 USD and passport fee of minimum 62 manat to be paid to Ministry of Internal Affairs over the online payment portal (http://www.gpp.az)
For people below the age of 18:
- Form (Download) (filled by parents if below the age of 15; filled and signed by the child if between the ages of 15-18)
- Original valid identification card (şəxsiyyət vəsiqəsi).
- A written consent of parents or legal representatives of children under the age of 18
- Original passport/identification cards (şəxsiyyət vəsiqəsi) and certificate of marriage of the parents of children under the age of 18
- If person under the age of 18 has been deprived of parental care, the consent of the adoptive parent, guardian or trustee;
- If person under the age of 18 has been deprived of parental care, original documents confirming the rights of the adoptive parent, guardian or trustee;
- If person under the age of 18 has been deprived of parental care, original passport or identification cards of the adoptive parent, guardian or trustee;
- 2 passport photos (3x4 or 3.5x4.5)
- Blood type certificate of the new born child
- Birth certificate (Apostille) of the new born child
- Consular fee: 25 USD and passport fee of minimum 17 manat to be paid to Ministry of Internal Affairs over the online payment portal (http://www.gpp.az)
Notes:
- Persons older than 18 must be present in person at the Consular Section of the Embassy.
- Persons under the age of 18 and his/her parents/guardians must be present in person at the Consular Section of the Embassy.
- Only complete applications will be accepted.
- The citizen must come to the Embassy in person with the above-mentioned documents. For this purpose, please contact the Consular Section of the Embassy in advance to schedule an appointment over the number (+965) 25 35 52 47.
